Armed with a clear understanding
of the full range of services that
are available and what options best
suit their personal circumstances
and aspirations, people with long-term
conditions will no longer have to
be passive recipients of services.They
will have the opportunity to be
active citizens, participating on
an equal footing with professionals
in determining what is right for
them.They will be able to choose
the right service for them, at a
time and place convenient to them
and delivered by the professional
best placed to offer that care.
By
the end of the next Parliament
Everyone with a long-term condition
should receive early, accurate and
expert diagnosis of their condition
and assessment of their needs.
Comprehensive
health information resources should
offer everyone equal access to high-quality
health information to help them
make decisions about their lives
with a long-term condition and about
their treatment and care. Such resources
should be developed with voluntary
organisations, taking advantage
of their substantial expertise in
this field.
Everyone with a long-term condition
should be offered a personal care
plan and should be offered a choice
of appropriate, convenient and accessible
lay-led self-management programmes.
