Information and support
People
with long-term conditions and their families and
carers need access to accurate, high-quality,
consistent information, in a range of formats
appropriate to their needs. They should be offered
support to guide them through shared decisionmaking
at every stage of their condition.
Despite
the plethora of health-related information in
the media, people with long-term conditions still
report difficulty in finding the information they
need, at the time they need it, and in an accessible
format. There are many examples of good practice
in the provision of information to patients, both
locally and nationally. It is time now to bring
these together to ensure a comprehensive system,
equally available to all.
By the end of the next Parliament
Comprehensive health information resources
should offer everyone equal access
to high-quality health information
to help them make decisions about
their lives. Such resources should
be developed with voluntary organisations,
taking advantage of their substantial
expertise in this field. |

Hadleigh
Practice Information Service
An
information service was set up
in Hadleigh Practice to provide
information and support in all
areas of health. On diagnosis,
patients can be given an ‘information
prescription’ from their
GP to inform them about the service
and how it can be accessed.
